The incumbent will contribute to the organization by taking responsibility for monitoring assessing and auditing health center performance and activities against patient safety standards and associated risks in the facilities Assist the Manager of Risk Management to design or manage the implementation of Risk Management related to client all clients including staff patients visitors and contractors safety and clinical competency within PHCC across all units Assist Manager of Risk Management to lead and support risk minimization methods development within PHCC and contribute to Patient Safety improvement initiatives as required Identify the areas of actual or potential risk as much as possible injuries to patients visitors and employees of PHCC and escalate to Manager of Risk Management Work with clinical and operational leads to devise policies and procedures and training programs to ensure physicians and clinical staffs provide a standardized service to patients Ensure that new initiatives and risk management trends for patient care are communicated fully throughout PHCC and Health Centers Establish and develop relationships with risk leads across Health Centers Support management in the reporting of incidents ensuring that learning is maximized and shared throughout PHC and Health Centre clinical staff Report clinical incidents internally and Lead system analysis to find contributing factors for system improvement Work closely with medical staff leadership clinical departments and committee chairs to ensure clinical staff follows up on risk governance infection control and safety Demonstrate proactive approach to patient safety by thinking what might go wrong in all undertakings and take necessary measures to prevent them from happening Identify and report potential or actual patient safety concerns errors and or near misses in timely manner Follow patient safetyrelated policies procedures and protocols Demonstrate a sense of team work and effective communication Partner with line manager to understand risks in the area of work develop a plan on how to mitigate those risks and develop shared accountability for implementing and evaluating the plan The incumbent will undertake any such appropriate duties or responsibilities as directed Ensure high standards of confidentiality to safeguard any sensitive information
