Job Description

  • Provides total individualized patient care and assumesaccountability for assessing, planning, implementing and evaluatingcare, discharge planning for each patient and assessing for currentand potential problems.
  • Accurately, conciselyand appropriately documents care, interventions andoutcomes.
  • Contributes to the achievement ofclinical quality outcome indicators and patient satisfactionbydemonstration of extraordinary service.
  • Utilizes a professional practice framework and evidencebased care, identifies improvement areas while minimizing riskscreating care that is optimal for our patients andfamilies.
  • Promotes a safe environment and safepatient care by embracing a safety culture and identifying areas ofrisk.
  • Educates the patient / family and othermembers of the health care team.
  • Contributesto excellence in outcomes, a positive patient experience and ahealthy professional environment.
  • Contributeseffective use ofresources.
  • Leads and / orcontributes to service based Performance Improvement activities andorganizational committees as assigned.
  • Develops and sustains (maintains) own knowledge, clinicalskills (mandatory training), professional (portfolio or profile)awareness and provides documented evidence of performance andmaintenance of skills consistent with position.
  • Treats colleagues, patients, and visitors with dignityand respect at all times and promotes and maintains effectivecommunication and teamwork.
  • Maintainsconfidentiality at all times.
  • Identifies andalerts members of the medical team to changes in medical status ofthe patient.
  • Works in collaboration with CaseManagement to achieve goals.
  • Participates innew staff / student orientation, assists in maintaining a suitablelearning environment and acts as a preceptor / mentor in thesupervision and teaching of new staff / students to develop theircompetence.
  • Maintains a professional demeanorand upholds the organizational values at all times.
  • Acts as a role model for junior staff and assists inpromoting and sharing knowledge via mentorship / preceptorshipprograms in line with relevant research and evidence basedpractice.
  • Follows all Hospital relatedpolicies and procedures.
  • Performs otherrelated duties as assigned.
  • Education :

  • Bachelor Degree in Nursing (4 years) or
  • Diploma in Nursing (3 years after post high school)and
  • Post graduate certificate in specialty ifapplicable in country of origin
