Job Description

Provides total individualized patient care and assumesaccountability for assessing, planning, implementing and evaluatingcare, discharge planning for each patient and assessing for currentand potential problems.

Accurately, conciselyand appropriately documents care, interventions andoutcomes.

Contributes to the achievement ofclinical quality outcome indicators and patient satisfactionbydemonstration of extraordinary service.

Utilizes a professional practice framework and evidencebased care, identifies improvement areas while minimizing riskscreating care that is optimal for our patients andfamilies.

Promotes a safe environment and safepatient care by embracing a safety culture and identifying areas ofrisk.

Educates the patient / family and othermembers of the health care team.

Contributesto excellence in outcomes, a positive patient experience and ahealthy professional environment.

Contributeseffective use ofresources.

Leads and / orcontributes to service based Performance Improvement activities andorganizational committees as assigned.

Develops and sustains (maintains) own knowledge, clinicalskills (mandatory training), professional (portfolio or profile)awareness and provides documented evidence of performance andmaintenance of skills consistent with position.

Treats colleagues, patients, and visitors with dignityand respect at all times and promotes and maintains effectivecommunication and teamwork.

Maintainsconfidentiality at all times.

Identifies andalerts members of the medical team to changes in medical status ofthe patient.

Works in collaboration with CaseManagement to achieve goals.

Participates innew staff / student orientation, assists in maintaining a suitablelearning environment and acts as a preceptor / mentor in thesupervision and teaching of new staff / students to develop theircompetence.

Maintains a professional demeanorand upholds the organizational values at all times.

Acts as a role model for junior staff and assists inpromoting and sharing knowledge via mentorship / preceptorshipprograms in line with relevant research and evidence basedpractice.

Follows all Hospital relatedpolicies and procedures.

Performs otherrelated duties as assigned.

Education :

Bachelor Degree in Nursing (4 years) or

Diploma in Nursing (3 years after post high school)and

Post graduate certificate in specialty ifapplicable in country of origin
