Coordinates and facilitatesdelivery of patient and family care within a multidisciplinaryframework primarily in an inpatient care setting, to ensure apatient / family focused and service oriented environment that is inaccordance with Evidenced Based and Best Practicestandards.
Under the supervision of theAttending Physician, assesses and identifies the health needs ofthe population and plans, implements, and evaluates interventionsappropriate to meet those needs in accordance to organizationalstandards and accreditation requirements.
Collaborates with multidisciplinary health care team todevise treatment plans based on patient / family needs to ensure thedelivery of high quality and evidence based services that aresufficient, efficient and responsive, and designed to meet theidentified patient / family care needs and organizationaloutcomes.
Contributes to ensuring thatclinical practices within the Division of Cardiology / Critical Careare in compliance with all applicable standards and requirements ofthe Joint Commission International, Ministry of Public Health, andall other applicable regulatory bodies.
Provides counseling and anticipatory guidance to patientsand families regarding physical, psychosocial and developmentalneeds.
Facilitates effective inter- andintradepartmental communication between patient, family, communityand interdisciplinary team members for collaboration, liaison,networking and teamwork to ensure the maintenance of strong andprofessional relationships.
Elicits, performsand records the history of physical examinations on assignedpatients. Under the supervision of the Attending Physician,develops a differential diagnosis for the presenting problem andimplements a plan for management.
Develops andperforms necessary diagnostic and therapeutic procedures perstandardized protocols.
Under the supervisionof the Attending Physician, collaborates with Case Management andDischarge Planning teams to plan and facilitate the continuity ofcomprehensive care of specialty patient populations from hospital,home and outpatient setting.
Assists in theassessment of learning needs of patients and families, and works intandem with the Community and Patient Education Department todevelop and implement patient education materials and programs thatare centered on the learning preferences, religious and culturalvalues, and appropriate reading and language skills of targetaudience to meet those identified needs.

Participates in the Quality andPerformance Improvement activities through regular chart review,targeted CQI activities, and the systematic and detailed review andevaluation of patient outcome data.
Providesboth day-time and, less frequently, night-time patient carecoverage as directed by the clinical schedule of theDivision
Provides effective and timely recordkeeping and documentation.
Maintains knowledgeof current research and incorporates findings into clinicalpractice.
Actively participates in clinicalresearch projects as directed.
